CCP Supervisor
Job Description
The Community Care Program Supervisor is responsible for overseeing and managing the operations of the Community Care Program. This role requires maintaining accurate records, supervising caregivers, and ensuring quality service delivery to clients while upholding organizational standards and compliance.
Roles & Responsibilities
- Data Management: Maintain electronic records for clients and caregivers, including communication notes, documentation, and electronic visit verification (EVV).
- File Maintenance: Ensure the organization and confidentiality of client and caregiver files, adhering to data privacy standards.
- Client Engagement: Conduct monthly calls, home visits, quarterly conferences, annual reviews, and other interactions to assess client needs and satisfaction.
- Caregiver Supervision: Recruit, hire, and train caregivers, providing ongoing in-service training and professional development opportunities.
- Compliance and Reporting: Ensure adherence to program policies, confidentiality requirements, and regulatory standards, preparing reports as needed.
Qualifications
- Education: High school diploma is required; a bachelor’s degree in social work, healthcare administration, or a related field is preferred.
- Experience: Previous experience in community care programs or supervisory roles is highly desirable.
- Skills: Proficiency in electronic data management systems, along with strong communication, organizational, and leadership skills.
